At the Premier Spine and Pain Center, we streamline our approach to your care. From the initial consultation to your follow-up appointments, we believe in keeping your time away from home to a minimum while providing you with more effective relief for acute and chronic pain.

Prior to Your Visit
What should I do before my first visit?
We know how painful it can be for you to spend time away from home. To help keep this at a minimum, we provide patient forms for download prior to your appointment. After you’ve made your appointment, please be sure you bring the following with you to your appointment.
What to Bring To Your Appointment
- New Patient Forms (You can download our new patient forms and fill them out ahead of time.)
- A record of what medications you have tried to alleviate your pain, including over the counter medications as well as medications you have been prescribed by other doctors.
- Previous x-rays, CT scans, and MRI films with you to the doctor. We may request previous records from other doctors you have seen.
- A list of your current medications, even those which are not for your pain condition. Also include any over the counter medications, dietary supplements, vitamins, or minerals.
What to Expect at Your Visit
At your initial visit, we will discuss your pain history, past pain treatments and potential pain management treatments. We believe in conservative care that will improve your quality of life, and we take the time to listen to your questions and concerns before prescribing treatments. We also offer a multidisciplinary approach, meaning that we work closely with primary care physicians as well as many specialists to optimally coordinate your care.
Follow-up Appointments
We will follow-up with you after your visit to monitor your pain and to schedule any additional appointments needed. Depending on your diagnosis, we may schedule a one-time follow-up or monthly follow-ups in order to continually and effectively manage chronic pain.
